Transitioning into a highly organized, multi-platform facility utilizing advanced tools like PointClickCare and Charity Tracker to manage data across specialized internal groups (including dedicated Case Management and TAR Request Teams).

Securing a vital partnership alignment with Partnership HealthPlan of California (PHC), allowing the facility to officially offer authorized Extended Care Management (ECM) Services and Community Support (CS) services.

Expanding outreach to tackle emergency eviction rapid responses, dedicated ER intake team, and community services to be added.
